Kentucky publishes no per-building quality result: each building’s scorecard sits behind the rate contractor’s provider login. Every point, add-on and net here is SuperLTC’s scoring on the Department for Medicaid Services’ published rules (State Plan amendment 25-0004 and 907 KAR 1:065) of CMS’s public long-stay quality measures, from the release the rule uses for each rate date, and of occupancy and Medicaid days from the Kentucky Office of Inspector General’s Certificate of Need annual long-term care report, standing in for the cost-report schedule Kentucky scores. Center of Excellence-NF participation is not public and is counted as none. Operators are CMS’s chain names; map points are CMS’s geocodes. No patient data appears here.
